Breast Implant Replacement When and how to intervene on implants over time

Breast implant replacement is a surgical revision procedure designed to update or correct the results of a previous breast augmentation surgery. Although modern implants are highly safe and durable, they are not meant to last forever. Over time, natural changes in the body or alterations in the implant itself can make a replacement necessary.

There are several reasons why this procedure may be considered. In some cases, it is due to a deterioration of the implant shell, such as wear, rupture, or a loss of cohesiveness of the internal gel. In other situations, patients may experience capsular contracture, which is a hardening of the fibrous capsule that naturally forms around the implant and can sometimes affect the breast’s shape or cause discomfort. Changes in the body, including weight loss, pregnancy, or breastfeeding, can also significantly impact the appearance of the breasts, leading patients to seek a revision. Additionally, some individuals simply wish to modify the aesthetic result, choosing a more natural profile or, on the contrary, a fuller and more projected look.

Before proceeding with surgery, the surgeon carries out a detailed clinical and instrumental evaluation. This often includes hormonal tests and ultrasound scans, which are essential to exclude any endocrine or pathological causes that might require a medical rather than a surgical approach.

Surgical options

The procedure for breast implant replacement varies significantly depending on the reason for the intervention. The main options are:

Simple replacement,

indicated when the periprosthetic capsule is healthy and there are no complications; it involves removing the old implant and inserting a new one, typically through the same original incision, thus reducing new scarring;

Capsulectomy or capsulotomy,

when capsular contracture is present; the first involves total removal of the fibrous capsule, while the second involves incising or releasing it to soften the tissues and improve comfort;

Implant position change

useful in cases where the implant is visible or palpable; the implant can be repositioned from a subglandular to a submuscular or dual-plane position, depending on tissue thickness and the desired result;

Combination with mastopexy,

when there is a desire to lift and reshape the breast in addition to replacing the implant, for example, after pregnancy, weight changes, or skin laxity.

During the procedure, the surgeon may also correct pre-existing asymmetries, redefine the lower pole profile, and adjust the implant volume to suit the new body balance.
The procedure is performed under general anesthesia, lasts about 1-2 hours, and typically involves discharge the same day or after one night of observation.

Recovery time and results

The post-operative course of implant replacement is generally faster than that of the initial breast augmentation. The patient wears a supportive bra for about 4-6 weeks, avoiding strenuous efforts in the first days.

Mild swelling or discomfort is normal and resolves spontaneously. Return to normal daily activities occurs within a few days, while sports and more intense chest movements should be avoided for about one month.

The results are visible immediately, but become fully stabilized after a few weeks, when the tissues adjust to the new implant. Depending on the shared decisions, the breast can appear more natural, toned, and proportionate compared to its current shape, better meeting the aesthetic and anatomical needs of the patient.

As with any surgical procedure, there are risks, although rare, such as infection, seroma, or slight asymmetry, but their incidence is minimal thanks to modern techniques and a careful prophylactic protocol.

Follow-up and prevention

After breast implant replacement, regular scheduled follow-ups are crucial to monitor the implant condition and the health of the surrounding tissues. Annual ultrasounds and, when necessary, MRI scans are essential tools for checking the implant integrity over time.

Next-generation implants, thanks to their cohesive silicone gel and advanced surfaces, ensure long durability and high stability over time. However, periodic evaluation remains essential for intervening promptly in case of changes or anatomical alterations.

Breast implant replacement is a revision and update procedure, allowing for the maintenance of a natural, harmonious, and safe breast. Every procedure is personalized based on the surgical history, body structure, and patient’s desires.

It represents not only an aesthetic improvement, but also a way to maintain continuity in personal well-being, because taking care of your body also means renewing self-confidence.
